What Are Wooden Nickels?

SMILE DESIGNERS WOODEN NICKELS are given to patients at their regular visits for areas of EXCELLENT COOPERATION. This is done to help all of our patients fulfill their treatment goals as efficiently as possible. It is determined at each visit how many nickels each patient receives by how the patient does on their COOPERATION EVALUATION. The patient can then save the nickels and redeem them for any prize that they desire, as long as they have the correct number of NICKELS for that prize. The prizes include movie passes, t- shirts, and gift certificates. This is our way of saying, "Thanks for being such a great patient." Every game or sporting event has rules. Here are ours:

A Smile Designers Wooden Nickel is Given If:

1. You arrive for your appointment on time.
2. Your toothbrushing was evaluated as good or better by our staff.
3. No braces, wires, or appliances are lost, broken, or loose.
4. You have worn your removable appliances or elastics correctly.
5. REFER - A - FRIEND - RECEIVE "20" WOODEN NICKELS!

Smile Designers Wooden Nickels Value & Prizes:

• You will have to option to "CASH-IN" your WOODEN NICKELS when you earn them or they can be saved for a future date.
• At no time can your WOODEN NICKELS be taken away from you, but cooperation at each appointment will determine how many WOODEN NICKELS are received.
• WOODEN NICKELS MUST BE TURNED IN TO REDEEM YOUR PRIZE!!
