All of our New Patients receive a Welcome "packet," which contains answers to many questions that you may have about our practice. When calling our office as a New Patient, you will have the option to have the information sent to you by email or regular mail. Below are the documents that should be filled out prior to your first appointment, this will make your New Patient experience as efficient as possible.
